Build custom questions
In this video, we will walk through how to build an effective review request form using custom questions. In particular, we will highlight how to collect both profile property information and product-specific feedback.

Next, keep the following considerations in mind as you build your custom questions. That way, you’ll implement a reviews strategy that accomplishes key goals for your brand.
Ask the right number of questions
While custom questions are incredibly beneficial, you should avoid overwhelming reviewers with too many. In general, only ask questions that will directly improve your marketing strategy and customer experience.
Best practices:
- Require no more than 3 questions.
The shorter your form is, the more likely someone is to fill it out. Do not mark every question as required. Instead, only do so for your most important questions. - Lead with your most important questions.
Reviewers are more likely to answer the first questions in your form, so keep in mind what your most important questions are and show them in order of importance.
Ask the right type of question
Choose from any of the following 4 formats when building your custom review questions:
- Single-choice:
Use when you want reviewers to select 1 option from a list of preset options. You can display choices in a click-able list or dropdown menu. - Multi-choice:
Use when you want reviewers to be able to select more than 1 option from a preset list. - Range:
Use when you want reviewers to select a number (1-5) to rate a particular product or experience. - Center range:
Use when you want reviewers to select an option from a range. Customize these options to fit your brand, setting a minimum, middle, and maximum value (e.g., light, medium, heavy).
Ask questions about a specific product or collection
Not all questions will apply to your full product catalog. That’s why you can specify when to ask a custom question: either for all products or just certain tagged products.
To only ask a question for a specific product or collection, add a tag to relevant products in Shopify.
Then, return to Klaviyo and build your question. When creating your custom question:
- Set this question to appear for Certain tagged products.
- Add your relevant tag to the Enter Shopify product tags field.
Ask private questions
Answers to private questions are not displayed publicly, so other shoppers will not see the answers that a past reviewer shared. Instead, answers to private questions will only appear in your Klaviyo account (on the customer’s profile for profile questions, and within the Submitted review event for product questions). That way, you can use this information to send more relevant content in the future.
